Update README now we use 74HC138

Replaced SPI IO chip with DIP. Also added extra debug header, and deleted Slot 6. Closes #49

Remove pull-ups on SD card page. Now the '138 drives all ~CS lines inactive (high), we don't need them. Besides pulling the whole bus just on the SD card page was weird. To pull-ups remain for write-protect and card detect, so they don't flap.

Retitle sheets and renumber parts.

Replace '245 with '138. Closes #65 and #70 and #58
