Minor AGND - PGND isolation modification

Updated motor connector pinout labels. Power stage moved closer to driver IC to minimize gate-line inductance.

Updated rev3.0 schematic with new signal/power header, correct library refrences for multi-footprint components, removed numerous test points. Initial pcb layout complete - rm 2 VCC rail caps, header prob points for motor.
